Rheumatoid factor is an autoantibody directed against IgG, it's a commonly requested test in the investigation of patients with joint problems. Although the test has been found to be both sensitive and specific for rheumatoid arthritis, and, in fact, the factor rheumatoid is included as part of the American College of Rheumatism 1987 revised criteria for the classification of rheumatoid arthritis and ACR/EULAR 2010 criteria. Its presence is associated with a poorer prognosis, and with extaarticular manifestations. However, the clinical utility of the test is not clear yet and the results can be overinterpreted and clinical decision made on this basis may be inappropriate. It's not a patognomonic characteristic of this diseases since positive rheumatoid factor may be found in others autoimmune disorders, in association with infection or oncologist disease and, even, in healthy people.
