Aging will reflect the dynamic and cumulative effects of time on the skin, soft tissues and deep structures such as retention ligaments and bone. They are the combined effects of gravity, progressive bone resorption, a decrease in the elasticity of the tissue and the redistribution of fat. Methods: An investigation consisting of a bibliographic review on the existing scientific literature has been carried out. A systematic search for books has been carried out and articles have been consulted in the main databases: Pubmed, Medline, and Scielo, selecting scientific articles from reviewed academic publications, and the following descriptors or keywords have been used: facial aging, agein , facial fat. Results: The review comprised 93 studies articles reviews. Divided into studies carried out in the different anatomical parts, skin, fat compartments, retention ligaments, muscles and bone structure. Aging would occur in a sequence of events. The earliest onset of aging probably results from a reduction in bone volume, both maxillary and mandibular, which follows a cascade of secondary changes due to the interconnection of the tissue layers. Consequently, the fat, preperiosteal and buccal, also has less projection Conclusions: Knowledge of the aging process helps in planning new cosmetic surgery techniques. As well as assessing the need for different treatment options for different degrees of aging. The great challenges of Aesthetic Medicine are not only to remove wrinkles, but to reposition and elevate the tissues.
