Parathyroid cysts are rare clinical entities. They are usually asymptomatic and occur incidentally during an imaging scan or during cervical cervicotomy. The definitive diagnosis was established after the demonstration of PTH in the intracystic fluid or using immunohistochemical techniques. Their epithelial origin, probably from the third and fourth pharyngeal pouches. Classically are classified as functioning (called pseudocysts associated with hyperparathyroidism) and non-functioning (not true cysts not associated with hyperparathyroidism). Two cases of parathyroid cysts in two male non-functioning. In both cases, the diagnosis was established by immunohistochemistry with positive marker for PTH in the epithelium of the cyst. We discuss the treatment options and proposed nomenclature of "parathyroid cyst associated with hyperparathyroidism" for the functioning cysts and "cyst parathyroid not associated with hyperparathyroidism" for the non-functioning for cysts.
